Two newspapers (New Albany Daily Ledger and Louisville Daily Union Press) dated April 15, 1865, including reports of President Abraham Lincoln's assassination.
These forms were filled out by people attending the city's bicentennial celebration on the Belvedere. The form was a survey of the people attending and gives their names, where they were born, ages, occupation and their attitudes and opinions of Louisville at the time.
